TRUE FACTS: IS TAPENTADOL A GLORIFIED TRAMADOL?

There are two similar drugs called Tramadol and Tapentadol. Tramadol was released in 1995 and Tapentadol was released in 2009. Both drugs are used as painkillers, but Tapentadol is a stronger drug than Tramadol.

Tramadol

Tramadol is a medicine used to treat pain. The enantiomers have different pharmacological effects: R-tramadol is responsible for the analgesic effect, and S-tramadol increases seizure risk. Tramadol is used for the relief of moderate to severe pain. It is sold under the brand names Ultram, Ralivia, Dromodol, and Tramal. The recommended total daily dose of Tramadol for adults is 25 to 100 mg. There is some controversy over the use of Tramadol during breastfeeding, so mothers who are taking this medication should be closely monitored for any signs of respiratory distress, feeding difficulties, or lethargy in their infant.

Tapentadol

Tapentadol is a special kind of medicine that helps to relieve pain. It works by affecting some of the chemicals in the brain, which helps to reduce pain signals from being sent to the brain. Tapentadol is available as a tablet, and the recommended dose for adults is 600-700 mg per day. It is not recommended for use in people with severe liver problems, but can be used with caution in people with moderate liver problems. Tapentadol has been shown to be effective in reducing pain after surgery, as well as in cancer-related pain and chronic low back pain.

Tapentadol is then excreted through the kidneys and has a half-life of approximately 4.25 hours.

If a patient is taking medication that inhibits the 2D6 enzyme or is classified as a ‘poor’ metabolizer of 2D6, they may experience decreased pain relief and metabolism. This is because 2D6 is responsible for breaking down Tapentadol into its active form, M1. Inhibitors or poor metabolizers of 2D6 can lead to a decrease in M1 levels by up to 40%. The most common 2D6 inhibitors are doxepin, citalopram, sertraline, and fluoxetine.

Tapentadol is a newer medication that is similar to Tramadol. It is a synthetic opioid that is used to treat moderate to severe pain. Tapentadol is a mu-opioid receptor agonist and also inhibits the reuptake of norepinephrine and serotonin. The average half-life for the M1 metabolite is 7.4 hours whereas the mean half-life of Tramadol is approximately 6 hours.
